From Monday 29th September, all requests for a GP appointment will need to be submitted by completing a special medical request form available on our surgery website Contact us about your request - Accurx Patient Portal or via the NHS app.

If you do not have access to the internet, or find it difficult to use, feel free to give us a call and we will complete the medical request form with you over the telephone.
